Equity Discount Mini Yoga Membership Agreement

By accepting this membership agreement, I acknowledge and agree to the following:

• I am enrolling in the Mini Yoga Membership at KTM Wellness LLC dba Moonflower Studio (“the Studio”)

• My membership will be billed at $54.00 per month, plus applicable processing fees.

• My membership will auto-renew each month on my original purchase date unless I request to cancel or freeze my membership in accordance with the terms below.

• This membership does not require a long-term contract and I may request membership cancellation or freeze anytime.

• Membership includes four (4) regularly scheduled weekly yoga classes per month.

• Workshops, events, and private appointments are not included unless otherwise specified.

• Membership includes a discounted rate of $13.50 for regularly scheduled weekly yoga classes on top of the four (4) allotted to my membership.

• I understand that additional classes will be charged to the payment method on file, plus applicable processing fees, either on the next billing cycle or by the front desk.

• Membership includes a discount on select workshops, typically $5 off.

• Membership includes 1 guest pass per month, to be used for new guests only. 

• To cancel my membership, I must submit a written request to info@moonflowerbk.com at least 7 days prior to my next billing date.

• Cancellations received less than 7 days before billing may result in one additional monthly charge.

• Membership freezes are available for a maximum of 90 days per year, either consecutively or non-consecutively.

• To freeze my membership, I must submit a written request to info@moonflowerbk.com at least 7 days prior to my requested freeze date.

• Membership freeze requests received less than 7 days before billing may result in one additional monthly charge.

• Refunds are not available for late or delayed cancellation or freeze requests. In certain circumstances, the Studio may offer account credits at its discretion.

• I understand that pre-registration for class is highly recommended.

• I may cancel a class reservation up to 2 hours before class start time without penalty.

• I understand that late cancellations (within 2 hours of class start) will result in loss of class credit.

• I understand that no-show for class will result in loss of class credit.

• I understand that I may not be admitted to class if I arrive late, and it will count as a no-show.

• Late cancellation and no-show fees will be charged to the payment method on file, plus applicable processing fees.

• I understand that class schedules and instructors may change, and classes may be canceled due to unforeseen circumstances.

• I understand that the Studio may suspend or terminate my membership for unsafe or disruptive behavior.

• This agreement represents the entire understanding between myself and the Studio, and continued participation indicates acceptance of any future updates.
